Sleep Disorders
Short-term benefits, long-term trade-offs
Sleep is one of the most common reasons people use cannabis. Short-term cannabinoid use can reduce sleep onset latency and increase slow-wave sleep, but tolerance develops quickly and rebound insomnia is well-documented on cessation. The long-term picture is more complex than popular accounts suggest.
What the Research Shows
Cannabinoids interact with sleep architecture through multiple pathways. THC reduces REM sleep and sleep onset latency acutely, while CBD appears to have a biphasic effect — low doses may be mildly stimulating, while higher doses promote sleep. The endocannabinoid system regulates circadian rhythms through CB1 receptors in the suprachiasmatic nucleus. Short-term clinical evidence is reasonably consistent: cannabinoids reduce time to sleep onset and subjective sleep quality improves. However, the 2023 meta-analysis of 14 RCTs found that these benefits diminish with continued use as tolerance develops, and withdrawal is associated with rebound insomnia, vivid dreams, and REM rebound. For sleep disorders secondary to other conditions (PTSD, chronic pain, MS), cannabinoids may offer more durable benefit by treating the underlying condition rather than sleep directly.
Key Findings
THC reduces sleep onset latency short-term
Well-StudiedMultiple RCTs confirm faster sleep onset with THC, but effect diminishes within 2–4 weeks of regular use.
REM sleep is suppressed by THC
Well-StudiedTHC consistently reduces REM sleep duration and density — implications for memory consolidation are unclear.
CBD has a biphasic effect on alertness
Emerging ResearchLow doses (15mg) may increase wakefulness; higher doses (160mg+) appear to promote sleep.
Rebound insomnia occurs on cessation
Well-StudiedRegular users experience worsened sleep, vivid dreams, and REM rebound for 1–3 weeks after stopping.

What We Still Don't Know
These are open research questions — areas where the evidence is insufficient or actively contested.
- 1What is the long-term impact of REM suppression on cognitive health?
- 2Can tolerance to THC's sleep effects be prevented or reversed?
- 3What is the optimal CBD dose and timing for primary insomnia?
- 4Do cannabinoids improve objective sleep measures (polysomnography) long-term?
- 5How do cannabinoids interact with standard sleep medications (benzodiazepines, z-drugs)?
